Qworks. This is a web-based survey application that can be customised to meet your data gathering requirements.  Here are the highlights of its capabilities: 

o        Form Building. This allows the user to design survey questionnaires, define data entry format and design rules.  With just a few mouse clicks, a web-based form designed according to the user’s specification could be created.

o        Email Circulation. The user would use this function send and manage email invitation and reminders to target respondents.

o        Data Tabulation.  A data analysis tool to help the user select, match, tabulate and export data collected through the web based form,
